Job Description – Community Support Worker


  • A Community Support Worker enables Service Users with learning disabilities to live a full and rewarding life within their own home/community
  • The key objective of the role is to provide the level of support required by each individual service user to fill the gap between what they are personally able to do and what they actually need to be able to do to lead an independent life
  • All job-holders are required to work within the framework of the individual service users care plans and shared action plans to ensure that all of their needs are met
  • Additionally, job-holders are required to assume a “Key Worker” role for a specific service user; assuming overall responsibility for their well being and acting as a vital link between the service user and all the other component parts of their life
