- Our dedicated Transition Manager oversees every aspect of moving new or existing services forward.
- We support Tenants to quickly settle into their new home with minimum disruption to their lives.
- We ensure risks are identified managed and minimised.
- Communication is key – we provide information packs about the transition to the tenants and their families.
- We work with the Tenants, their families and advocates to carry out Needs Assessment for each individual Tenant.
- We liaise with Housing Providers to ensure that the properties meet Tenants’ needs.
- We ensure that all staff are trained to meet specific needs of the people they support / will be supporting.
- We work closely with outgoing providers to ensure that the transfer of staff is carried out effectively and sensitively.
- We liaise with all stakeholders to finalise the moving in arrangements.
